Patient’s first office visit

After you schedule your first visit, you will receive a copy of our new patient packet, which will contain forms:

1. Medical History, and
2. Quality-of-Life Questionnaire.

Also included will be a Bladder Diary to record your drinking and urination habits and any problems you might have in controlling your urine. The Bladder Diary takes 24 hours to complete, so be sure to allow yourself enough time before your appointment.

Please bring the completed forms to your first office visit.
DO NOT MAIL THESE FORMS.

A list of the items you will need to bring with you to your first visit follows:

1. Completed Medical History Form
2. Completed Quality-of-Life Questionnaire
3. Completed Bladder Diary. Please follow the directions provided. A standard one cup measuring device may be used to measure volume in ounces or cc's.
4. A current list of your medications or your medication bottles.
5. Old medical records, operative reports, and urine culture results that might be pertinent to your condition.
6. Your health insurance card(s).
