Sperm Banking

The Fertility Center has now been offering sperm banking services for greater than 20 years. 

Why sperm banking?

Patients elect to bank sperm for a number of reasons such as:

  • for fertility preservation, prior to radiation therapy or chemotherapy; prior to a vasectomy;
  • to increase total number of sperm available for use in procedures like artificial insemination or in vitro fertilization;
  • before extended military duties during which time a spouse is attempting to conceive
  • for other recommended or personal reasons

Requirements

The basic requirements for semen banking at University Women’s Healthcare – Fertility Center include:

  1. Scheduling an appointment at (502) 271-5845.
  2. Registering with the University of Louisville Healthcare system, if you have not done so already.  This can be done by calling (502) 271-5062 and indicating you need to register prior to sperm banking.
  3. A written order from your physician for semen banking.  This may be brought in at the time of the appointment or faxed to the attention of the IVF Lab at (502) 271-5984. A copy of this form can be obtained in the Patient Forms section.
  4. Current blood testing for HIV I and II, Hepatitis B antigen (HBV) and Hepatitis C antibody.  These tests should be completed prior to banking, however, in cases where limited time is available, samples may be collected and held in an isolation tank until results are known.  In the event of a positive test, semen samples must be stored at an offsite location.
  5. Completion of the Sperm Cryopreservation and Storage Agreement form, (Available in Patient Forms).  This form should be read prior to arriving for your first appointment for sperm banking.  At the beginning of the appointment a member of the Fertility Center staff will meet with you to answer any questions you may have about the agreement and witness your signature at that time.
