Payment Policy
Payment for service is due in full at the time of service.
- We accept cash, check, Visa, MasterCard and Discover
- All fees are based on the type of service provided.
- If the patient is a minor (18 years or younger), the parent or guardian is responsible for payment of the account in accordance with the policies outlined above.
- For elective or uncovered surgical services; all co-payments and deductibles are due before your surgery.
- When an account becomes more than 90 days overdue, it will be referred to an outside collection agency. We refer to an agency only after we have exhausted efforts to arrange for voluntary payment. Collection and court costs will be added to the patient's account.
- You must bring any required referrals for treatment at or before the scheduled appointment. If you do not have a referral, your visit could be rescheduled or you could be financially responsible.
Medicine is both a science and a professional art. On that basis, our group's physicians must grapple with the complexity of establishing their fees for any given procedure. They determine fees by referring to the:
- Difficulty and complexity of the procedure,
- Length of time required to make a diagnosis and outline treatment in the most efficient and effective way, and
- Cost of medical supplies.
