Becoming a Patient

APPOINTMENT: To become a patient, you must first make an appointment. Call 502-561-8850 to make arrangements to be seen by one of our physicians.

PATIENT REGISTRATION: On your first visit, please register with our receptionist upon your arrival.

WHAT TO BRING: In order to establish your medical and insurance records, it's necessary that you bring a copy of your insurance card and bring a photo identification card, such as your driver's license.
