Planning your first visit
- Call 502-561-8850 to make an appointment
- Bring all current presciption bottles or a list of all prescriptions you are currently taking to your first visit
- Bring any insurance or Medicaid or Medicare cards
- If another doctor has referred you, please bring any referral to your appointment
- If you have any health records from other physicians, bring those as well.
What to expect on your first visit
PATIENT REGISTRATION We ask first-time patients to register
with our receptionist. In order to establish your medical and insurance
records, it's necessary that you bring a copy of your insurance card and
bring a photo identification card, such as your driver's license.
THE TREATMENT PROCESS Depending on the nature of your
condition, you may need to be seen by more than one health care professional.
Because of our affiliation with the University of Louisville School of
Medicine, fellows, residents and medical students in training may occasionally
participate in your treatment. These knowledgeable professionals are an
integral part of our health care delivery team and offer additional insights
and observations to assist you. Laboratory or other diagnostic tests may
also be recommended.
FOLLOW-UPS
If a follow-up visit to our offices is necessary, you can make arrangements
with our receptionist before you leave.
